A model validated in 677,000 people across low-, moderate- and high-risk regions in Europe.

SCORE2 combines six factors — age, sex, smoking status, blood pressure, total cholesterol and HDL cholesterol — in a competing-risk survival model that estimates the 10-year probability of a fatal or non-fatal cardiovascular event.

The calculator applies the official coefficients for low-risk regions (which include Spain and the United Kingdom). The result is then compared against the thresholds defined in the 2021 ESC Guidelines on Cardiovascular Disease Prevention: low, moderate or high.

For people aged 70 and above the appropriate model is SCORE2-OP, a dedicated variant for older adults. Below the age of 40 the model does not apply; in that case, lifetime risk should be discussed with your physician.

Stay on course.

Healthy habits, annual blood pressure and lipid checks, and a periodic preventive review are usually enough at this stage.

Moderate · 5–<10%

Address the modifiables.

It's worth working on smoking, blood pressure and cholesterol under medical supervision. A cardiovascular imaging review may add useful context.

High · ≥10%

Detailed assessment.

A full cardiovascular work-up is indicated — echocardiogram, retinography, advanced lipid panel — alongside a discussion of preventive treatment with your physician.
